Education Services. SEN Assessment.
37 hours per week. Fixed Term until 31/03/2022 (Funding Limited Project).
£33,799 to £39782 per annum (Grade J).

We are seeking an Assessment Coordinator from January 2020. This post is offered on a fixed term contract until 31 March 2022, on a full or part time basis.
The role will be to coordinate the new Education Health and Care assessments. Coordinators work very closely with families throughout the process, including meeting families in their homes, chairing multi agency person centred planning meetings and developing Education Health and Care Plans with the young person and their family.
You will need a sound working knowledge of SEN and disability and a background in education, health or social care. Commitment to person centred and personalised approaches are essential, as are excellent interpersonal skills and the ability to create high quality outcomes-based plans.

This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2017; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
